How To Join

There are no appointments necessary to join Elemental. Stop by anytime during our business hours from 9am-9pm seven days a week to begin your free membership. You can also submit your membership agreement by using the Intake Form on our website. This allows us to contact your doctor and verify your patient status before you come in, saving you time, and ensuring you can enjoy all the privileges of membership on your first visit without delay, including amazing free gifts on your first 3 visits! You can even join using our digital membership agreement, and once we verify your profile, you can place a home delivery order!

Becoming a Member is Easy

Before you can begin enjoying membership privileges you are required to provide us with documentation verifying that you are at least 21 years of age. In San Jose, medical patients must also be at least 21 years old.

Stop by anytime during our business hours to register for your free membership or to save time, register online. Online registration allows us to verify your documentation ahead of time, so you can enjoy all the privileges of membership on your first visit and to place an online order without delay!

What to Bring

When you arrive, we will ask you to present your government issued driver’s license or identification card.

We do not currently accept passports.

Free Gifts

Please enjoy a complimentary $15 credit when you spend at least $30 (subtotal) on your first visit.

Medical Renewals

If you renew your Doctor's Recommendation, please bring your embossed Renewal (sorry, photocopies not accepted) plus one of the forms of identification listed above. If you need help obtaining a doctor recommendation, visit our Resources page. To be exempt from state taxes you will also need to provide a valid MMIC card.

Primary Caregivers

Please provide your original embossed Doctor’s Recommendation (sorry, photocopies not accepted) PLUS one of the forms of identification listed above.
